Alcohol and Drug Screening, Education, & Relapse Prevention Proposal

Siloam International and David Jones

1. Program Description:

A. Siloam International, under the direction and lead counselor/consultant David A. Jones MA, will provide individual and group education regarding prevention and intervention for students at risk or involved in substance use or abuse. The objective is to foster skills for making healthy choices around chemicals which are potentially addictive in nature. This is a model built upon public health concepts and the medical model of addiction as a chronic and progressive disease, which, if left unchecked, is likely to result in serious health and social consequences for the individual, their family, and society.

B. I have over ten plus years as a senior counselor in the area of drug and alcohol education. In addition I am educator and trainer of groups. My focus is on addiction, violence, and culture. I have worked with adults and youth in schools, prisons and community programs. I have worked on programs in the U.S. as well as internationally with the United States government in places such as Russia, Estonia, Slovakia, and Moldova in the area of drug reduction and treatment methods for newly independent states of the former Soviet Union.

2. General Program Services:

A. I will provide both alcohol and drug screening interviews, which are based on the addiction severity index, capturing history and use of individuals. I maintain complete control over such material and instruments. As an independent contractor for several organizations, and as an individual private practice counselor, the necessary security regarding client history and level of treatment are standard professional practices of my work.

B. All services will be provided at agreed upon locations by the contractor and client, to be arranged prior to start of contract.

C. Strategies to motivate clients to participate will be developed on an as-needed basis once the population is in class. Based upon experience with culturally and economically different client groups around the world, such factors as age, race, gender and class make for different methods of motivation, rewards and consequences employed to gain objectives and compliances.

3. Budget:

Typical Hourly Rate: $65.00/hour for all individual work, including assessment, counseling, and documentation. This is my standard rate for individual substance abuse counseling.

Typical Hourly Rate for Group Treatment: $250.00/hour for groups up to 8 clients.

Typical Hourly Rate for Educational Groups for Prevention and Education of Substance Abuse Problems: $200.00/hour.
